PLEASE NOTE: High School students must also submit a copy of their credit counselling summary, which is available from their guidance counsellor at the school they are enrolled in. (If you are from another school board, please fill out these forms and indicate your school board on the form.)

Important note: If you wish to obtain high school credits, you must also submit the summer school registration form.

All chaperones and/or supervisors who are not active TCDSB employees must compete the Volunteer Sector Screening and Registration form.
